CRDS de l’Ouest

The Centre de répartition des demandes de service (CRDS) de l’Ouest provides primary care physicians in the districts of CSSS Cavendish, de Dorval-Lachine-LaSalle, de la Montagne, de l’Ouest-de-l’Île and du Sud-Ouest–Verdun with a single access point to refer patients who require specialized services.

Someone from the appointment service at CRDS de l'Ouest phones the patient within 72 hours of receiving a Request for services form to confirm that it is being processed. The patient is then given an appointment based on his or her clinical priority.

Currently, primary care physicians can refer their patients for two specializations: cardiology and ophthalmology. However, visit this section regularly since the services offered by the CRDS de l'Ouest is constantly changing and other specializations will be gradually added.

Mécanismes d'accès Pédiatriques (MAP)

I'm a primary care physician and I want to refer a patient to pediatric medicine

Mechanisms to refer patients to general pediatrics (Mécanisme d’accès pédiatriques - MAP) have been put in place in order to facilitate access to pediatric specialists (including general pediatrics) for family physicians and for pediatricians in the community.  

Specialists in the three hospitals providing specialized care for children in the region of Montreal participated in the project: Ste-Justine Hospital, Montreal Children's Hospital of the McGill University Health Centre and Maisonneuve-Rosemont Hospital.

By using the request form, the referring physician will be informed and guided through the referral process. This request form proposes:

  • Clinical prerequisites
  • Clinical priorities
  • Complete clinical information for the consulting physician

Primary advantages for the referring physician

  • Quick management of the referral request
  • Ensurance that the service request will be treated according to the clinical priority
  • Response by fax to the referring physician within 72 hours to confirm receipt and an appointment within 7 days post-consultation
  • Clinical nurse as a resource for the referring physician and the patient/family member

Send a request for a consultation

  1. Print the request form for the specialized service requested
  2. Complete the form, sign it and include your permit number
  3. Send the service request by fax to the number indicated on the service request form
  4. Give the  «information sheet» to the family members / patients

CRDS de l’Ouest is an initiative of the Agence de la santé et des services sociaux de Montréal and of the CSSS and hospitals in the west of Montréal. The Centre is coordinated by CSSS du Sud-Ouest–Verdun.
